Evidence matrix

A structured record separating expectation, observation, impact, reproducibility and decision.

Working definition

An evidence matrix prevents impressions from becoming facts or isolated failures from becoming general product verdicts. Each finding receives context, severity, evidence and an owned follow-up decision.

It supports pilots, security reviews and incident retrospectives. A score can show trends but cannot replace a reproducible finding or a justified stop condition.
